10A NCAC 39A .1206. PATIENT APPLICATION PROCESS

Patients may apply for breast and cervical cancer screening and follow-up services at any participating local health department or at the office of any other participating provider. A copy of the list of participating local health departments and other providers may be obtained from the Division of Public Health, 1915 Mail Service Center, Raleigh, North Carolina 27699-1915at no cost.
