14B NCAC 08B .0104. APPLICATION PROCESS: STEP THREE: DOCUMENTS

  • (a)  The chief will inform each applicant to return the completed applications for employment to him with the following documents:

    (1)           Completed Fingerprint Cards.  The applicant will be provided two fingerprint cards with instruction to fill in required information and take the cards to a law enforcement agency.  The applicant will request authorities at the agency to record his/her fingerprints on the cards.  The completed cards will be returned with other documents to the chief for mailing to the State Bureau of Investigation (SBI).  Upon receipt of the cards, the SBI will make an instate and a Federal Bureau of Investigation check for any criminal record.  Upon completion of the check, the SBI will return one of the cards, along with any record found on the applicant, to the chief and will retain the other card and a copy of the record for its own files.  The chief shall file the fingerprint card in the applicant's file.

    (2)           High School Diploma or General Education Development Certificate. An applicant must have received a diploma from an accredited high school or hold a General Education Development Certificate approved by the North Carolina State Board of Education.

    (3)           A transcript of the applicant's high school grades and the grades from any educational institution beyond high school which the applicant might have attended.

    (4)           Applicant's birth certificate.

    (5)           A recent 3" x 5" head and shoulder photograph.

    (6)           Completed Medical History Form.  The applicant will be provided this form.

    (7)           Completed copy of the Personal History Questionnaire.  The applicant will be provided this form.

    (8)           Vision Test Results.  The chief will direct each applicant to have vision tests administered by a driver's license examiner.  An applicant must perform on the test according to standards established by the BPS Medical Services Director.  Vision requirements are as follows:  minimum 20/50 corrected to 20/20 in each eye with corrective lenses.  An applicant with less than 20/20 vision must possess and wear lenses which correct his/her vision to 20/20.  An applicant who is colorblind, suffers from night blindness, or has depth perception problems will not be accepted for membership in BPS. If corrective lenses are required, they must be in the possession of the applicant during the initial screening, STEP FIVE, as identified in Regulation .0106 of this Section.

    (9)           A copy of the applicant's military separation papers, if applicable.

    (b)  If a completed application is returned to the chief without all required supporting documents identified in this Regulation the official date of the application for the chronological list will not be fixed until such time as the documents are received by the chief.
