20 NCAC 02N .0217. APPLICATIONS FOR DISABILITY BENEFITS

To receive disability benefits under G.S. 58‑86‑55, a member must submit a disability determination application. This application asks the member's physician and fire chief or rescue squad captain whether they can certify:
(1) that the member is totally and permanently disabled;
(2) that the disability resulted from extreme exercise or activity experienced in the course and scope of the member's official duties; and
(3) that the disability has required the member to leave his fire department or rescue squad.
Upon receipt of a disability determination application the office of the director shall submit it to the board at their next meeting.
