21 NCAC 06N .0110. FORM BAR‑9

Latest version.

(a) Form BAR‑9 must be filed when one desires to open or manage a new barber school. It requires information such as, but not limited to, the following:

(1)           name and address of school;

(2)           name and address of each manager;

(3)           name and address of instructors;

(4)           physical dimensions of the school; and

(5)           number of barber chairs and lavatories.

(b) Form BAR‑9 must be notarized.

(c) Form BAR‑9 must be accompanied by a fee according to G.S. 86A‑25.
